checkboxes to include build profiles

Hello all,

  I am wondering if it is possible to have check boxes in a request form that will include build profiles or exclude them based on the status of the check box.  I know that I could use check boxes and pass them out to external VCO workflows based on the status of the box, that is probably going to be the end game, but looking for a way to use check boxes now.  I have been scouring documentation and I have not found a way yet.  Can someone lend a hand please?
