Request form empty when requesting a Appd cataog item.

This one's driving me crazy from 2 days.

So i have an App director which is publishing an application to my vCAC server. When i try to request it, it just ends up with a blank white screen. Tried almost everything from re-registering the App director to rebooting all the servers involved.

And when i click on the item it throws an internal error.
